Zangrillo torna a parlare dell’emergenza Covid in Italia. Alberto Zangrillo, responsabile dell’Unità operativa di Terapia intensiva generale e Cardiovascolare dell’Irccs ospedale San Raffaele di Milano e prorettore dell’università Vita-Salute San Raffaele, ha rilasciato una intervista a Il Foglio. Ecco uno stralcio delle sue parole riportate da ADNkronos.com: “Il virus c’è, ma serve un’assunzione di responsabilità”.
“Noi abbiamo sperato che la seconda ondata fosse più blanda, ma ci sono considerazioni molto importanti da fare a monte”.
Zangrillo intervista Il Foglio 10 novembre
La prima, per Zangrillo, “riguarda questa continua evocazione della guerra e di scenari terrificanti da parte di persone che hanno ruoli istituzionali, cosa a mio avviso controproducente per via dell’effetto panico che provoca. La uso anche io, ora, per spiegare: che cosa prevede la battaglia? Se la seconda e la terza linea alzano bandiera bianca, si arriva a combattere solo nel fortino – che sarebbe l’ospedale. E questo è il limite: le persone, spaventate, non seguite a domicilio, arrivano tutte in ospedale. Un iper-afflusso, visibile dai codici di accesso”.
Per l’area metropolitana di Milano, dice Zangrillo, “nelle settimane tra il 23 ottobre e il 6 novembre, tra le persone arrivate in ospedale (gruppo San Donato) il 60-70 per cento sono codici bianchi e verdi, e il tempo medio di dimissione è di 21 ore. In questa situazione, i pazienti, in Pronto soccorso, coabitano con quelli arrivati per patologie non Covid e con quelli che hanno una sintomatologia Covid intermedia o grave, cioè i codici gialli e rossi. Questo porta alla saturazione”.
Per liberare il Pronto soccorso, allora, “bisogna passare per un lavoro di rimodulazione dei ricoveri in reparto, per poter dedicare più letti ai pazienti che non possiamo dimettere subito. In un sistema organizzato, questo è il punto, molti tra i pazienti che necessitano di ossigeno-terapia potrebbero essere gestiti anche a domicilio”.
“L’altro drammatico disguido, la corsa al tampone: oggi una persona, appena scopre di avere una linea di febbre, cerca spasmodicamente di verificare se ha contratto il virus o meno, spesso non riuscendoci in tempi brevi. E questo fa perdere tempo prezioso: la battaglia contro Sars-CoV-2 passa dalla diagnosi e dalla terapia medica tempestiva”.
“Ogni medico deve essere allora in grado, ed essere messo in grado, di assumersi la responsabilità del caso, prima di tutto parlando con il paziente per cercare di catturare elementi utili a escludere il ricovero, e procedere alla cura domiciliare, o per indirizzare al contrario il paziente in ospedale. Le persone sono spaventate, ma vanno seguite a casa prima di tutto”.
E però da più parti si invoca il lockdown: “Il lockdown in alcuni casi può essere indispensabile, ma bisogna leggerlo come fallimento. Vediamo già un 30 per cento in meno di pazienti con patologie non correlate al Covid: molti rimandano esami e visite. Ma spesso non c’è tempo, e questo avrà un prezzo. Non si può vivere di sola medicina difensiva”.
